I'd like to review my application with someone because once again I was rejected and I'd like to know why and what I can do. I'm applying as undergrad and I know a handful of applications weren't even considered last year because requirements were not met so I want to make sure that I'm not one of those.

You could probably email admdvm to ask for some feedback. They might be flooded with messages right now, though.
Review the course requirements on the website to make sure your application was valid in the meantime if that is something you suspect could be the problem (ie. Ensure you took valid required prereqs, you took a full courseload, 60% of courses were from 3rd/4th year level, etc)

They likely won't give you individual feedback. If you can verify your academic requirements are met (via the website, there are no tricks) and your GPA was admissible then you need to start looking at your references. Be 100% confident you got good references and listed enough hours working with the referees so the admissions committee can see they know you well enough to give an accurate reference.

I'll email them again. I sent in my courses to have them approved and it all came back saying they were good to use and I'm going to assume that they check the semesters they're in since they want the transcript. If I knew what it was then I'd change it because I'm considering taking Masters but if it's a waste of time I'd like to know.

Don't know what to tell you. The requirements are spelled out plainly. They would only ever toss your application if you were in the top 200 (marks-wise under acceptable conditions) if you had a red flag. I don't believe previous application attempts are even looked at so it should be straightforward to figure out what the issue is.
Doing a Masters will allow you to apply from a different pool (if the problem is your undergrad marks or structure this may help), but will not exempt you from the reference requirements (if they are your flag then it would be a waste of time). Everyone who is rejected wonders where they went wrong. I don't blame the OVC for not giving individualized feedback to several hundred students.

I had a problem with a few of my prereqs (I didn't have time to send them for evaluation) and they contacted me after I submitted them asking if I can send them any substitute courses. After changing one curse and providing an explanation for others, the courses were approved and yesterday I got an invitation for interviews. We were emailing back and forth for almost a week and it looked like the admission counselor really wanted to help and to find a solution for me. So I guess that if you had had a problem with your prereqs they would've contacted you too. I think if your GPA is high enough, then you should look into your references.

I can empathize. My average was in the 90s, and I didn't get an interview. There must have been a red flag somewhere in my app but I genuinely don't know what happened

Same here. I have an 86 with prereqs and last two semesters so it could just be the cutoff was higher this year.

Most of the points were already covered but I really think it should be emphasized how important your references are. Your referees must be able to confidently speak to your abilities as THEY have assessed them by working/spending time with you. If you have not spent much time working directly with someone (i.e. just worked/volunteered in the clinic at the same time without direct interaction/supervision), they are likely not an appropriate reference. It's also important that they provide a strong positive reference, not just an average one. Statistics for grade cut-offs etc. will come out in August, but in the meantime generally for interviews it's somewhere in the mid-high 86s. If you fall somewhere in that area or below, chances are that grades were the reason. Things always come down to decimal points, which is unfortunate but it's the nature of the beast. Those are the two main things I would look at. In September there is usually a session held for unsuccessful applicants which you can watch via live stream or attend in person, where these types of things are discussed and participants can ask questions (individual feedback will not be given though). I would email admissions to have your last two semesters and course pre-requisites APPROVED. That's what I did before applying (it turned out, one of my semesters was inadmissible and I had no idea) That way you can rule out the 60/40 rule and/or overlapping content. inadmissable
Did you references get their letters in on time?
Were your grades within range? Last year, the cutoff was around 85 for an interview and the class average was just above 90.
